You must provide the following:
 

*Name & Phone Number| Date & Time| Location & Duration|

*Picture of Photo ID & A photo of you holding your ID near your face.

 *Link to your business site (if you have one)
 

*A business card (if you have one)

*Names & Contact information of at least 1 or 2 established providers you can use as a reference.

 

*A Deposit of 30% for all new friends.  (50% deposit for new friends with no references)
